Ahava Therapy Services in Tennessee is seeking an Occupational Therapist to evaluate, plan, and implement individualized treatment plans to maximize patients' functional independence.

You will work closely with nursing and therapy teams, educate families, supervise therapy assistants, and participate in discharge planning. This on-site role emphasizes continuous education, HIPAA-compliant documentation, and compassionate care.

Qualifikationen

• Bachelor’s or Master’s Degree in Occupational Therapy.
• Active OT license in the state of practice.
• BLS/CPR certification (current or obtained during orientation).
• Strong clinical reasoning, communication, and documentation skills.
• Ability to collaborate effectively within an interdisciplinary team.

Aufgaben

• Review clinical information and assess its impact on the assessment and treatment process.
• Conduct comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
• Revise treatment plans based on patient performance and progress.
• Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care.
• Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
• Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
• Assist with discharge planning and maintain knowledge of community resources.
• Participate in care conferences, meetings, and quality improvement initiatives.
• Maintain clinical competence through continuing education opportunities.
• Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.
• Support the company’s mission, vision, and professional expectations.
• Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.
