Job summary

Ahava Therapy Services is seeking a PRN Occupational Therapist (OT) in Whites Creek, TN, to join a collaborative, in-house rehab team at Whites Creek Wellness & Rehab. The role focuses on comprehensive assessments, personalized treatment plans, and ongoing patient progress in an interdisciplinary setting.

You will conduct functional evaluations, document outcomes, and contribute to quality care while maintaining regulatory standards and efficient use of therapy resources.

Qualifications

• Bachelor’s or Master’s degree in Occupational Therapy.
• Active Tennessee OT license.
• Current BLS/CPR certification.
• Strong observational, verbal, and written communication skills.
• Ability to work collaboratively within an interdisciplinary team.

Responsibilities

• Review resident information and assess impact on treatment.
• Conduct and document functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable goals.
• Maintain timely and accurate departmental records and reports.
• Enter progress notes in the resident record after each session.
• Supervise assistants and orient residents to services.
• Collaborate with nursing, residents, and families to maximize function.
• Assist in referrals and post-discharge community resources.
• Attend rehab team conferences and pursue continuing education.

Responsibilities

• Review available resident information and evaluate the impact on assessment and treatment.
• Conduct and document thorough functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Comply with evaluation, treatment, and documentation guidelines; maintain timely and accurate departmental records.
• Enter treatment and progress notes in the resident record following each session.
• Revise treatment plans as needed based on patient progress.
• Document the course of treatment in clear, concise discharge summaries.
• Instruct Assistants in treatment procedures and provide supervision as needed.
• Orient residents to program services and treatment procedures.
• Collaborate with nursing staff, residents, and families to maximize functional abilities.
• Establish and instruct residents and caregivers on exercises and therapeutic procedures for post-discharge carryover.
• Assist in the referral process for additional services after discharge; maintain knowledge of community resources.
• Attend and participate in rehabilitation team conferences and other departmental or facility meetings.
• Access continuing education opportunities to maintain clinical currency.
• Participate in quality improvement processes and respond to audits, surveys, safety activities, and regulatory requirements.
• Act in compliance with Ahava Therapy Services’ regulatory and professional standards.
• Adhere to facility policies and participate in quality improvement and safety programs.
• Perform other duties as assigned to provide quality care.
• Understand reimbursement by various payers and allocate therapy resources efficiently.
• Demonstrate knowledge of OBRA regulations, HIPAA, Local Coverage Determination Policy, and third-party reimbursement guidelines.
• Support the company’s mission and vision and project a professional image.
• Independently perform duties and recognize situations where direct supervision is needed.
• Maintain expected productivity levels.
