Job summary

Ahava Therapy Services is seeking a Full-Time Occupational Therapist to join the Maplewood Healthcare team in Jackson, TN. The OT will evaluate, treat, and discharge patients, collaborating with an in-house rehab group to maximize functional outcomes.

We look for candidates with a degree in OT, an active license, and CPR/BLS certification. The role emphasizes clinical excellence, documentation, and teamwork in a setting with short-term rehab, long-term care, and outpatient therapy services.

Qualifications

  • Bachelor’s or Master’s Degree in Occupational Therapy.
  • Active OT license in the state of practice.
  • BLS/CPR certification (current or obtained during orientation).
  • Strong clinical reasoning, communication, and documentation skills.
  • Ability to collaborate effectively within an interdisciplinary team.

Responsibilities

  • Review clinical information and evaluate its impact on assessment and treatment.
  • Conduct comprehensive functional assessments using standardized procedures.
  • Develop and implement individualized treatment plans with achievable goals.
  • Document evaluations, daily treatments, progress notes, and discharge summaries.
  • Revise treatment plans based on patient performance and progress.
  • Supervise and guide therapy assistants, ensuring proper techniques.
  • Educate residents, families, and caregivers about exercises and procedures.
  • Collaborate with nursing, interdisciplinary team members, and families.
  • Assist with discharge planning and community resources knowledge.
  • Participate in care conferences, meetings, and quality initiatives.
  • Maintain clinical competence through continuing education.
  • Adhere to HIPAA, payer guidelines and facility standards.
  • Support the company’s mission, vision, and professional expectations.
  • Maintain productivity levels and demonstrate teamwork and communication skills.

About the Facilities
