Job summary

Ahava Therapy Services is seeking a licensed Occupational Therapist to join our Clarksville team. You will evaluate patients, design functional treatment plans, and document progress with clear communication to families and care teams.

You’ll collaborate with nurses and therapists, educate caregivers, participate in discharge planning, and pursue ongoing education to maintain clinical excellence in a compassionate setting.

Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy.
• Active OT license in the state of practice.
• BLS/CPR certification (current or obtained during orientation).
• Strong clinical reasoning, communication, and documentation skills.
• Ability to collaborate effectively within an interdisciplinary team.

Responsibilities

• Review clinical information and evaluate its impact on the assessment and treatment process.
• Conduct comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
• Revise treatment plans based on patient performance and progress.
• Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care.
• Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
• Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
• Assist with discharge planning and maintain knowledge of community resources.
• Participate in care conferences, meetings, and quality improvement initiatives.
• Maintain clinical competence through continuing education opportunities.
• Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.
• Support the company’s mission, vision, and professional expectations.
• Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.
