Asbury Solomons, part of Asbury Communities, is seeking a Registered Nurse in Maryland to lead nursing care coordination at our 40-bed waterfront facility. You will oversee MDS processes, care planning, and cross-disciplinary collaboration to ensure high-quality resident care.

Formación

• Must be a Registered Nurse licensed in Maryland.
• Two years of long-term care experience.
• Experience with PointClickCare preferred.
• Strong supervisory and leadership skills.
• Proficient computer skills for data entry and reports.

Responsabilidades

• Conceives and creates care plans based on comprehensive resident assessments.
• Schedules PPS/MDS assessments within established time frames.
• Leads the assessment coordination process for nursing sections of MDS assessments.
• Compiles data for resident insurance updates (PPO and managed care payer sources).
• Communicates with Director of Therapy and Insurance Case Managers to maximize RUG levels for reimbursement.
• Leads the MDS completion process across responsible disciplines.
• Works with the interdisciplinary team to coordinate care plans/assessments.
• Supervises nursing staff and participates in Nurse Management on-call rotation.
