• LTCFS certification required.

• Strong written and verbal communication skills.

Responsibilities

• Schedule home-based LTCFS assessments.
• Use LTCFS to assess member abilities and health status.
• Coordinate with ICT and family to complete LTCFS.
• Maintain adherence to LTCFS timeframes.
• Submit documentation per guidelines and attend trainings.
