Job summary

Molina Healthcare in Wisconsin is seeking a hybrid role to support long-term care screening and coordination for older adults and adults with disabilities. The candidate will travel to member locations and meet at the Molina office once a month.

Qualifications

• At least 1 year health care experience supporting older adults or disability.
• LTCFS certification and active licensure where required.
• Bachelor’s degree in a health-related field or social work; active license if applicable.
• Valid driver’s license with reliable transportation.
• Ability to support staff development and balance team dynamics.
• Experience with data management, project coordination and training.
• Strong written and verbal communication skills.
• Proficiency in Microsoft Office.

Responsibilities

• Schedules appointments for LTCFS assessments with members.
• Gathers information on functional abilities and health using LTCFS.
• Completes LTCFS in collaboration with member, family and ICT.
• Coordinates with ICT to ensure enrollment, annual and condition-change screens.
• Ensures data consistency between assessments and member records.
• Uses spreadsheets to track LTCFS timeframes.
• Submits timely, accurate documentation per guidelines.
• Attends meetings and trainings to stay updated on processes.
• Participates in quality assurance and routine audits.
• Maintains LTCFS certification through CST testing.
• Local travel may be required for state/contractual needs.
