Molina Health care in Wisconsin is seeking a field-based Long-Term Care Functional Screen (LTCFS) Specialist. This hybrid role requires travel to member locations and monthly meetings at a Molina office.

You will conduct LTCFS assessments, collaborate with the member, family, and interdisciplinary care team, and ensure accurate documentation and timely data entry. A Wisconsin LTCFS certification and a related degree or licensure are required.

Qualifications

• LTCFS certification is required for the role.
• Bachelor’s degree in a social work, human services or health-related field or valid nursing/licensed credential if state dependent.
• Valid driver’s license and reliable transportation for travel.
• Strong communication and data/documentation skills.

Responsibilities

• Schedules LTCFS assessments with members at home.
• Uses the Wisconsin LTCFS to gather member information.
• Completes LTCFS with member, family and ICT.
• Coordinates with ICT for enrollment and condition screens.
• Ensures data consistency with member records.
• Tracks LTCFS timeframes with spreadsheets.
• Submits accurate documentation per guidelines.
• Attends meetings and trainings to maintain knowledge.
• Participates in QA and audits.
• Maintains LTCFS certification via CST.
• Local travel may be required.
