Heritage Inn Statesboro is seeking a part-time LPN to support patient care and care planning. You will coordinate with nursing staff, families, and consultants to ensure timely and accurate assessments and care plans.

The role requires a Georgia-registered LPN and proficiency with Microsoft Office tools. Flexible scheduling and on-site work are available, with opportunities for paid time off and holiday benefits.

Competenze

• Nursing degree or LPN program is required.
• Active LPN license to practice in Georgia is required.

Mansioni

• Facilitates communication between nursing center departments and patient/family for effective assessment/care plans.
• Assists Director with patient/family interviews and evaluation of patient conditions.
• Coordinates development and review of care plans and MDS processes with the interdisciplinary team.
