Job summary

NextStep Care in Georgia is seeking a dedicated Licensed Practical Nurse to join our Care Management Services team as a member liaison and program advocate. You will conduct member assessments, coordinate with multidisciplinary teams, and help develop care paths with family involvement to meet member goals.

In this role you will observe home environments, screen referrals, document data, and support optimal health outcomes.

Qualifications

• Valid Georgia LPN license with 2 years of experience in health and human services.
• Experience in social work, home and community based services, healthcare or geriatrics preferred.
• Valid Driver’s License.
• Reliable transportation.

Responsibilities

• Observe members in their home environment gathering medical, environmental, and psychosocial data for review by the multidisciplinary team.
• Accepts referrals and ensures potential members are screened in a timely manner as required by organization and governmental policies.
• Addresses member or caregiver concerns effectively and efficiently.
• Assist in carepath developments, recommending HCBS services and incorporating roles for informal caregivers.
• Records and documents member information completely and accurately in accordance with federal, state, and operational guidelines.
