Job summary

NextStep Care Georgia is seeking a dedicated care manager to act as a liaison between members and home and community-based providers, ensuring quality and cost-effective care and outcomes. You will collaborate with primary care providers, managed care plans, HCBS teams and families to promote access to services and coordinated care.

This role requires regular member contact through calls and home visits, accurate documentation of interventions, and development and adjustment of care plans.

Qualifications

• Bachelor's degree in social work or related human services field is required with 2 years of work experience.
