Job summary

NextStep Care in Georgia is seeking an LPN to serve as a Care Management Services member liaison, ensuring program and member goals are met through primary care enhanced case management. A primary function is completing member assessments.

You will observe members at home, screen referrals, address concerns, assist in carepath development, document data, and collaborate with a multidisciplinary team to remove barriers to care.

Qualifications

• Must hold a Georgia LPN license and have 2 years of experience in health and human services.
• Experience in social work, home and community based services, healthcare or geriatrics preferred.
• Valid Driver's License and reliable transportation required.

Responsibilities

• Serve as the Care Management Services member liaison and representative to ensure program and member goals are met.
• Complete member assessments.
• Observe members in their home environment and collect medical, environmental, and psychosocial data.
• Screen referrals and ensure timely eligibility and screening per policy.
• Collaborate with the multidisciplinary team to develop care paths and coordinate services.
