Job summary

Visiting Nurse Health System in Marietta, GA is seeking a Care Manager LPN to assess, coordinate, and monitor community-based services for elderly or disabled clients under the CCSP. You will perform home visits, reassess care needs, and help develop plans of care that support safe transitions home.

This role requires collaboration with physicians, social workers, and caregivers, documentation of care plans, and adherence to VNHS standards and DCH policies.

Qualifications

• LPN program graduate with Georgia license and CPR certification.
• 2 years of nursing experience in community health, long-term care, or geriatrics.
• Familiar with Medicare/Medicaid regs and care coordination.

Responsibilities

• Conduct in-home assessments to determine services.
• Collaborate with interdisciplinary teams on LOC decisions.
• Develop care paths and discharge plans based on client needs.
• Document assessment tools and care plans per policy.
• Coordinate hospital-to-home transitions and follow-up.
• Maintain confidential client records and participate in meetings.
