Job summary

Enhabit Home Health & Hospice is seeking a qualified intake/coordinator professional to support referrals and patient transitions in Dallas, TX. This role collaborates with branch and sales teams and ensures compliance with regulatory guidelines.

We require a high school diploma, healthcare experience, knowledge of Medicare guidelines, and strong customer service with comfort using mobile devices. A nursing license is preferred for some hires.

Qualifications

• Must possess a high school diploma or equivalent.
• Must have at least two years of experience in the health care industry and one year experience in home health or pediatrics.
• Must have demonstrated experience in customer service.
• Must have demonstrated knowledge of Medicare guidelines that govern a home health or hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Education and experience, preferred: nursing license holder is preferred; experience with intake responsibilities is a plus.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure adequate coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
