Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Kingsport, TN. This PRN role requires supporting the Physical Therapist and delivering high-quality patient care in a team-focused environment.

Responsibilities include screenings, assisting in evaluations, developing treatment plans, and conducting individual and group sessions. Will also involve documentation in NetHealth EMR and training residents and families on equipment

Qualifications

• Associate's degree in Physical Therapy is required.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide PT services under supervision of Physical Therapist.
• Screen residents to determine intervention needs.
• Assist in evaluation and treatment planning.
• Help in establishing resident schedules with PT.
• Conduct individual and group therapy sessions as directed.
• Train residents and families in use of equipment.
• Ensure timely and accurate documentation in CMS-compliant EMR.
