Qualifications

• Graduate degree in nursing and NP licensure in practice state.
• Board certification in Adult/Family GP nursing specialties.
• Current BLS/ACLS or CPR certification.

Responsibilities

• Conduct adult/geriatric assessments including history and treatment plans.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP on education and follow up.
• Provide services at members' residences, including homes and SNFs.
