Job summary

Matrix Medical Network provides in-home health assessments led by NPs, reviewing medical history, medications and social factors to close care gaps. Providers perform assessments at members' homes or SNFs, with documentation per Matrix guidelines and collaboration with physicians and care teams.

Qualifications

• Must hold NP licensure in state of practice with prescriptive authority.
• Board certification in an acute or primary care NP specialty.
• Excellent communication and ability to work with diverse teams.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform adult/geriatric health assessments and document findings.
• Administer point-of-care tests and coordinate referrals.
• Collaborate with PCPs, case managers and caregivers for follow-up.
• Provide care at members' homes or SNFs as needed.
