Job summary

Envision, Inc. in Wichita, KS seeks a Targeted Case Manager to provide person-centered case management for individuals with IDD, ensuring services align with KDADS/HCBS guidelines and conflict-free case management standards.

You will coordinate supports, advocate for client goals, and maintain compliant documentation. The role emphasizes collaboration with families, providers, and community partners, requiring strong communication, organization, and problem-solving abilities while upholding

Qualifications

• Bachelor’s degree in a human services field preferred; equivalent experience considered.
• Minimum of six months working with individuals with developmental disabilities.
• Knowledge of IDD services, HCBS, and person-centered planning preferred.

Responsibilities

• Develop and maintain Person-Centered Support Plans (PCSPs) with the individual served and their support team.
• Coordinate medical, behavioral health, educational, residential, employment, and community supports.
• Monitor services to ensure needs are met and supports align with the approved PCSP.
• Advocate for the rights, dignity, preferences, and goals of the individual served.
• Assist individuals and families with accessing community resources, Medicaid, Social Security benefits, and waiver services.
• Maintain regular communication with individuals served, guardians, providers, and team members.
• Support transitions, referrals, discharges, and coordination of services.
• Complete accurate and timely documentation, billing, and case records.
• Report incidents and concerns according to agency and regulatory requirements.
• Participate in required meetings, trainings, and quality assurance activities.
• Maintain confidentiality and professional standards in accordance with HIPAA and agency policy.
• Flexibility to perform other duties as assigned.
