You will perform face-to-face, goal-directed interventions and help clients identify strategies, access services, and remain in their natural communities. You will document services in the EHR, coordinate with treatment teams and supports, ensure required trainings are completed, and support crisis planning to restore stability

Qualifikationen

• BA/BS degree or four years of equivalent education/experience in human services.
• Must pass background checks and state-specific screening requirements.
• If using a personal vehicle, have reliable, insured transportation compliant with laws.

Aufgaben

• Provide face-to-face, community-based CPST interventions as determined by the individualized treatment plan.
• Assist the patient in identifying strategies or treatment options.
• Provide individual supportive counseling, solution-focused interventions, emotional and behavioral management, and problem behavior analysis.
• Assist patients in addressing functional deficits by developing goals and utilizing strengths and needs.
• Assist patients in remaining in their natural community and address crises with a plan to restore stability.
