Job summary

RTI Dakota County is seeking a Targeted Case Manager to assess needs, develop service plans, connect clients to resources, and monitor progress for individuals with serious mental illness and co-occurring conditions. The role emphasizes community integration, client-centered planning, and coordination across services.

You will work to ensure safe, stable home environments, with regular meetings and thorough documentation, while upholding a respectful, inclusive workplace.

Qualifications

• Bachelor’s degree in a behavioral science or related field (or equivalent MN licensure).
• Meets the Minnesota Mental Health Practitioner definition or equivalent.
• Pass a Department of Human Services background check.
• Have a valid driver’s license and access to a working vehicle with auto insurance.
• 2,000 hours (1 year) of experience delivering services to persons with mental illness (preferred).
• Experience with targeted case management and commitment proceedings (preferred).

Responsibilities

• Assess needs, identify goals, and plan service so individuals’ needs are met and goals are advanced.
• Communicate consistent information and aid the team to ensure continuity of service.
• Document services in individual records and maintain standards.
• Provide flexible, individualized support for self-directed living.
• Access community and housing resources to match needs and preferences.
• Coordinate communication among involved parties to direct services.
• Provide crisis assessment and intervention to keep individuals safe.
• Promote an inclusive, diverse, culturally competent workplace.
• Meet with clients regularly and coordinate services.
• Manage civil commitments and prepare court reports.
