Job summary

Center seeks a Service Coordinator to support adults and children in IDD programs across Archer, Clay, and Wichita counties. The role includes crisis prevention, assessment, monitoring, and advocacy, with ongoing discovery and plan revisions based on client needs.

Qualifications

• Must have a bachelor’s or advanced degree in a social, behavioral, or human service field.
• Ability to communicate effectively in writing and orally.
• Experience with documentation and reporting in care services.
• Familiarity with LIDDA principles and community-based service delivery.

Responsibilities

• Provide crisis prevention and management, screening and assessment, monitoring, consulting, and advocacy.
• Maintain monthly contact targets with individuals and LARs/guardians.
• Document service delivery, outcomes, and satisfaction in the Person Directed Plan.
• Advocate with schools and attend ARD meetings as needed.
• Travel to caseload locations and coordinate with providers and agencies.
