Job summary

Envision, Inc. is seeking a Targeted Case Manager (TCM) to provide person-centered case management for individuals with IDD in Kansas.

You will coordinate medical, behavioral health, educational, residential, employment, and community supports to promote health, safety, independence, and community inclusion. Responsibilities include developing PCSPs, advocating for client rights, and ensuring timely documentation and compliance with KDADS and HCBS guidelines.

Qualifications

• Bachelor’s degree in a human services field preferred; equivalent experience considered.
• 6 months minimum experience with individuals with developmental disabilities.
• Strong communication, organization, and problem-solving abilities.
• Valid driver’s license and reliable transportation.
• CPR/First Aid certification or ability to obtain within 30 days.
• Must pass KDADS background checks including APS/CPS, KBI, MVR, OIG.

Responsibilities

• Develop and maintain Person-Centered Support Plans (PCSPs) with the individual.
• Coordinate medical, behavioral health, educational, residential, employment, and community supports.
• Monitor services to ensure needs are met and aligned with PCSP.
• Advocate for rights, dignity, preferences, and goals.
• Assist with accessing Medicaid, Social Security benefits, and waiver services.
• Maintain regular communication with individuals, guardians, providers, and team.
• Support transitions, referrals, discharges, and service coordination.
• Complete accurate documentation, billing, and case records.
• Report incidents per agency/regulatory requirements.
• Participate in meetings, trainings, and QA activities.
• Maintain confidentiality per HIPAA and policy.
• Flexibility to perform other duties.
