Job summary

Help USA is seeking a Case Manager for New York City’s Homebase program in the Bronx. You will provide case management, crisis intervention, and advocacy to clients facing housing instability and risk of shelter entry.

• Bachelor’s degree in social work or related field or equivalent experience.
• Strong verbal and written communication, plus negotiation skills.
• Excellent organizational and documentation abilities.
• Ability to take initiative, be dependable, and handle multiple tasks.

Responsibilities

• Conduct initial intake interviews and thorough needs assessments to determine eligibility.
• Develop service plans leading to crisis resolution and self-sufficiency.
• Provide referrals for employment, training, and education.
• Advocate for clients to access entitlements and housing stabilization.
• Coordinate with Housing Court Liaison for court advocacy and relocation needs.
• Prepare financial assistance requests to resolve housing crises.
• Participate in outreach to increase program enrollments.
