AccentCare, Inc. in Beaumont, TX is seeking a Hospice Clinical Social Worker (MSW) to join our compassionate team and support patients and families with dignity and empathy. You will collaborate with the interdisciplinary group to assess social, spiritual, and emotional needs and help plan care.

Required is an MSW and state licensure; Medicare/Medicaid experience preferred; grief counseling and discharge planning are a plus; some travel to local locations may be required. Full-time with benefits.

Qualifications

• MSW from an accredited school; valid licensure to practice social work.
• Medicare/Medicaid/insurance knowledge preferred.
• 1+ year post-graduate social work in healthcare; 2+ years preferred.
• Grief counseling experience preferred.
• Discharge planning experience preferred.
• Licensed to practice as a clinical social worker in the applicable state.
• Must be a licensed driver able to travel to all locations.

Responsibilities

• Assist patients and families as part of the hospice interdisciplinary team.
• Assess social, spiritual, and emotional needs to inform care plans.
• Prepare patients and families for changes and advance directives planning.
• Utilize community resources to support patients and families.
