Job summary

AccentCare, Inc. is seeking a dedicated Hospice Social Worker (MSW) in Wesley Chapel, FL for a full-time, on-site role. You will collaborate with an interdisciplinary team to support patients and families at the end of life, ensuring dignity and quality of life.

Requires an MSW and licensure to practice in Florida, with 1+ year in healthcare social work preferred. Experience in grief counseling and discharge planning is a plus.

Qualifications

• Master’s Degree from accredited school of Social Work; valid license/registration to practice in state if required.
• Medicare/Medicaid/Insurance specialty preferred.
• At least one year of professional social work experience (post-graduate) in healthcare; 2+ years preferred.
• Experience or education in grief counseling preferred.
• Discharge planning experience preferred.

Responsibilities

• Assist the core members of the hospice team in understanding social, spiritual, and emotional factors related to the patient’s health to establish a plan of care.
• Participate as a member of the interdisciplinary team in the plan of care for all patients.
• Assess social, spiritual, and emotional needs to estimate capacity and coping related to terminal illness.
• Prepare the patient and family for changes and support advance directives/advanced care planning.
• Utilize resources to support patient and family within illness limitations.
• Support the bereavement program.
• Provide discharge planning related to level-of-care changes.
