Job summary

AccentCare, Inc. is seeking a full-time Social Worker / MSW for hospice care in Kendall, FL and surrounding areas in South Miami. This on-site role supports patients and families, participates in the interdisciplinary team, and emphasizes discharge planning and bereavement program involvement.

The position requires an MSW, state licensure, and at least 1 year of healthcare social work experience (2+ years preferred). Travel to multiple locations is expected.

Qualifications

• Master’s Degree from accredited school of Social Work; valid license/registration to practice in the state(s) of operation
• Grief counseling experience or education preferred
• 1 year of professional social work experience in healthcare required; 2+ years preferred
• Licensed to practice as a clinical social worker in the operating state; valid driver and ability to travel to all locations

Responsibilities

• Assist core hospice team in understanding social, spiritual, and emotional factors influencing patient care to establish a dignified plan of care
• Participate as a member of the interdisciplinary team in planning and reviewing care for all patients
• Assess social, spiritual, and emotional needs of patients and caregivers to support coping with illness and daily living
