AccentCare is seeking a Full-Time Social Worker / MSW for Hospice in Largo, MD. This role focuses on holistic patient care, psychosocial support, and coordinating with the interdisciplinary team to ensure dignified end-of-life planning.

The candidate will have a Master’s in Social Work, state licensure, and experience with healthcare settings; grief counseling and discharge planning are preferred. AccentCare offers comprehensive benefits and a supportive environment.

Qualifications

• Master's Degree from accredited school of Social Work or related field.
• Licensure/permit to practice in state(s) of operation.

Responsibilities

• Assist the core hospice team in understanding social, spiritual, and emotional factors to establish a care plan.
• Participate in the interdisciplinary team and review the plan of care for patients.
• Assess social, spiritual, and emotional needs to estimate coping capacity of patients and caregivers.
• Prepare patient and family for advance directives and care planning.
• Utilize community resources to support patient and family.
• Support the bereavement program.
• Provide discharge planning related to level-of-care changes.
