Job summary

AccentCare is seeking a full-time Hospice Social Worker (MSW) in Beaumont, TX. This on-site role covers Mont Belvieu, Dayton and Liberty areas and involves collaboration with an interdisciplinary hospice team to support patients and families through effective care planning and discharge processes.

Ideal candidates have an MSW, current licensure, and 1–2+ years in healthcare social work, with Medicare/Medicaid insurance familiarity preferred.

Qualifications

• MSW from an accredited program; valid license/permit to practice as required by state law.
• Experience in hospice/social work preferred; Medicare/Medicaid/Insurance familiarity advantageous.
• At least 1 year post-graduate healthcare social work experience; 2+ years preferred.

Responsibilities

• Support patients and families by assessing social, emotional, and spiritual needs to inform care planning.
• Collaborate with interdisciplinary team to develop and review patient care plans.
• Provide education on advance directives and care planning; assist with discharge planning and community placement.
