Qualifications

• Must hold LCSW license in Illinois (IDFPR).

• Master’s degree in Social Work required.
• Excellent communication and crisis intervention skills.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems.
• Develop a bereavement plan of care addressing family needs and preferences.
• Interpret and respect patient/family faith and culture in care planning.
• Educate patients and families to overcome barriers and align with learning capabilities.
• Document assessments and interventions in the EMR and coordinate with the IDT.
• Assist with advance directives, referrals, and transportation/barrier matters.
