Qualifications

• Master’s degree in Social Work is required.
• Licensed clinical social work licensure in IL (LCSW) or equivalent.
• Licensed social worker (LSW) in IL is a valid credential.

Responsibilities

• Provide psychosocial assessments of patients and families to identify needs.
• Develop a bereavement plan of care addressing family member needs.
• Interpret and communicate patient/family faith and cultural traditions.
• Educate patients and families to overcome barriers and meet learning needs.
• Document assessments, interventions, and care plans in the EMR.
• Collaborate with IDT to develop and modify social work plans of care.
