Qualifications

• Master’s degree in Social Work is required.
• Licensed Social Worker (LSW) and/or Licensed Clinical Social Worker (LCSW) in Illinois.
• Driver’s license and proof of auto insurance required.
• Experience in hospice care and psychosocial assessments preferred.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop a bereavement plan of care addressing family needs.
• Interpret and communicate patient/family faith and cultural traditions.
• Educate patients and families to overcome learning barriers.
• Document assessments and care plans in the EMR and coordinate with IDT.
