Qualifications

• Master’s degree in Social Work is required.
• Licensed Clinical Social Worker (LCSW) in Illinois is required.
• Licensed Social Worker (LSW) in Illinois is an acceptable credential when applicable.

Responsibilities

• Perform psychosocial assessments to identify emotional, social and environmental needs of patients and families.
• Develop a bereavement plan of care and support family/caregivers.
• Coordinate with interdisciplinary teams to address patient’s psychosocial concerns and care planning.
