Qualifications

• Master’s Degree in Social Work required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) required.
• Illinois licensure (IDFPR) or ability to obtain licensure in Illinois.
• Valid Driver’s License and auto insurance.
• Hospice work experience preferred.

Responsibilities

• Perform psychosocial assessments of patients and families to identify emotional, social, and environmental needs related to diagnosis and life situation.
• Develop and implement a bereavement plan of care addressing family/caregiver needs.
• Educate patients and families on cultural and faith considerations and support self-determined care decisions.
• Document psychosocial assessments and interventions in the electronic medical record; coordinate care with IDT.
• Assist with transportation barriers and referrals for food, clothing, power bills, Medicaid, SSDI, and home care needs.
