Job summary

CenterWell Home Health is seeking a licensed Speech-Language Pathologist to deliver in-home therapy, assess swallowing, voice, language and cognitive function, and develop individualized plans of care with interdisciplinary teams.

You will evaluate patients in their homes or facilities, document progress, educate families, and participate in discharge planning while maintaining the highest professional standards and patient confidentiality.

Qualifications

• Certification of Clinical Competence in Speech-Language Pathology by ASHA (CCC-SLP) or Audiology.
• Minimum six months of experience as a speech-language pathologist.
• Current and unrestricted professional license.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe, record and report changes in the patient's condition to supervisor and/or physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation and progress reports as required by policy.
