Job summary

Enhabit Home Health & Hospice in Georgia is seeking a scheduling coordinator to align patients with field clinicians and ensure smooth care coordination across teams.

You will manage on-call scheduling, liaise with patients, physicians, and caregivers, and maintain hospitalization logs to support timely and accurate patient care. This role requires LPN licensure, CPR, and at least one year in clinical/home health settings.

Qualifications

• High School Diploma or equivalent is required.
• LPN or LVN license required.
• Valid state clinical license per requirements.
• Minimum 1 year of clinical healthcare experience.
• CPR certification required (or equivalent).
• Minimum 1 year of home health, hospice, or pediatric experience in last 24 months with staffing/scheduling experience.

Responsibilities

• Schedule patients to field clinicians.
• Coordinate with field staff, patients, physicians, referral sources, caregivers and other providers to maintain care coordination and continuity.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
