Job summary

Amedisys is seeking a registered nurse case manager in Stockbridge, GA to deliver high-quality home health care. You will assess patients, collaborate with the care team, and implement individualized plans of care to support healing at home.

Qualifications

• Minimum 1+ year clinical RN experience.
• Current RN license for assigned state(s).
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.
• RN Intern program if <6 months experience.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and self-care.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager.
• Plans and provides staff education.
• Performs on-call responsibilities and services as assigned.
• Participates in clinical development and continuing education.
• Other duties as assigned.
