Job summary

Amedisys seeks a Registered Nurse Case Manager in Duluth, GA to deliver comprehensive home health care. You will assess patients, develop and monitor plans of care, and coordinate with the care team to ensure optimal outcomes in a home setting.

Responsibilities include education, on-call duties, and supervision of LPNs and HHAs. Commitment to quality, documentation, and ongoing professional development is essential.

Qualifications

• 1+ year of clinical RN experience required.
• Current RN license for assigned state(s).
• Current CPR certification required.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Provides staff education and on-call coverage as assigned.
• Participates in continuing education programs.
