Amedisys in Carrollton, GA is seeking a Registered Nurse Case Manager to join our home health care team full-time. You will assess patients, develop and implement care plans, coordinate with the care team, and supervise other clinical staff to promote independent, safe living at home.

Requirements include at least 1 year of RN experience, an active RN license for the state, CPR certification, and a valid driver's license. Competitive pay from $77,000 to $96,000 per year plus benefits.

Qualifications

• 1+ year of clinical RN experience.
• Current RN license for the state.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsabilités

• Performs patient assessments and develops a plan of care with the care team.
• Makes referrals as needed and documents rationale.
• Promotes patient independence through teaching and rehab.
• Supervises LPNs and HHAs.
• Completes timely, accurate documentation.
• Communicates patient progress to the clinical manager.
• Plans staff education and participates in on-call duties.
• Participates in clinical development and continuing education.
• Other duties as assigned.
