Job summary

Amedisys Home Health in Georgia is seeking a dedicated Registered Nurse Case Manager for full-time work in the home health setting. You will assess patients, coordinate care with the interdisciplinary team, and implement plans that promote healing at home.

Qualifications

• 1+ year of clinical experience as an RN, with demonstrated patient care.
• Current RN license valid for the state(s) assigned.
• CPR certification maintained and current.
• Valid driver's license with reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patient's needs.
• Promotes patient health and independence through teaching and self-care techniques.
• Supervises LPNs and HHAs as part of the care team.
• Completes documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plans and provides staff education and participates in on-call duties.
• Engages in clinical development and continuing education programs.

Education

Registered Nurse (RN) license

1+ year clinical RN experience

CPR certification

Valid driver's license
