Job summary

Amedisys is hiring a Registered Nurse Case Manager in Georgia to join our home health care team. You will assess patients, develop and implement plans of care, and coordinate with the care team to promote healing at home.

Qualifications

• 1+ year of clinical RN experience
• Current CPR certification
• Valid driver's license and transportation

Responsibilities

• Performs patient assessments and collaborates with the care team to plan and implement a care plan
• Makes referrals as needed based on patient needs and documents rationale
• Educates patients to promote health and independence and teaches self-care techniques
• Supervises LPNs and HHAs
• Completes timely and accurate documentation
• Communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• Performs on-call duties as assigned
• Engages in ongoing clinical development and education
