Job summary

Beth Israel Lahey Health is seeking a physical therapist to plan, organize, and direct home care services within patients' homes and communities. You will coordinate care, perform initial assessments, and develop patient-centered plans of care, including preventive and rehabilitative actions, with patient and family involvement.

Requirements include a DPT or equivalent, MA PT license, BLS, and 2+ years in home health or public health.

Qualifications

• Degree from an accredited Physical Therapy program; DPT preferred.
• Massachusetts Physical Therapist license (current).
• Active BLS certification.

Responsibilities

• Coordinate care for assigned patient caseload, supervise PTA staff if applicable, and manage OASIS time points.
• Complete initial patient and family assessment to plan a patient-specific plan of care.
• Initiate and adjust the plan of care; administer therapies as prescribed and document outcomes.
