Beth Israel Lahey Health seeks a licensed Physical Therapist to join our Home Health team serving communities in Massachusetts. You will coordinate patient care, supervise PTA staff, complete OASIS evaluations, and develop individualized plans of care.

The role requires 1–2 years of PT experience (public health/home health preferred), MA licensure, active BLS, and a valid driver’s license. You’ll participate in case conferences, discharge planning, and ensure high-quality outcomes while

Qualifications

• Massachusetts Physical Therapist license required.
• DPT preferred; active BLS certification.
• 1–2 years PT experience; home health/public health preferred.
• Valid driver's license and reliable vehicle.

Responsabilités

• Coordinate patient care for a caseload including delegation to PTA staff.
• Supervise PTA staff and document care and OASIS time points.
• Develop and revise patient-specific plans of care with family involvement.
• Administer treatments as prescribed and counsel patients and families.
• Provide health instructions and ensure timely documentation.
• Attend case conferences and coordinate with other disciplines.
