Job summary

Beth Israel Lahey Health is seeking a Physical Therapist for home health to plan, organize, and direct services in patients' homes and communities. You will coordinate care, perform assessments, and collaborate with families and physicians to develop individualized plans of care.

The role requires a PT degree, NH license, BLS certification, and 2+ years in PT, preferably with home health or public health experience. OASIS familiarity is preferred.

Qualifications

• Degree from an accredited program in Physical Therapy.
• DPT preferred.
• Current valid NH Physical Therapist license.
• Active American Health Association BLS certification.
• 2+ years of physical therapy experience, at least one in public health or home care.
• OASIS experience is preferred.

Responsibilities

• Coordinate care for assigned patient caseload and supervise PTA staff.
• Complete initial patient assessment and develop plan of care.
• Initiate and revise plan of care as patient status changes.
• Administer medications and treatments as prescribed.
• Communicate with physician and document OASIS.
• Maintain HIPAA confidentiality and timely documentation.
