Job summary

Beth Israel Lahey Health seeks a dedicated Home Care Physical Therapist in Beverly, MA to plan, coordinate, and direct home-based therapy services for individuals and families in their homes and communities.

The role requires a MA PT license, active BLS, and 2+ years of PT experience, preferably in home care or public health, with OASIS experience preferred to support compliant, patient-centered care.

Qualifications

• Degree from an accredited Physical Therapy program; DPT preferred.
• Massachusetts PT license (active).
• Active BLS certification.
• 2+ years PT experience, with public health or home care preferred.
• OASIS experience preferred.

Responsibilities

• Coordinate care for home care patients, including supervising PTA staff (if applicable).
• Complete initial patient assessment and plan of care with patient and family.
• Initiate and revise the plan of care; administer therapies as prescribed.
• Communicate with physicians regarding patient needs and changes in condition.
• Complete OASIS documentation in accordance with timeliness policies.
