Job summary

Beth Israel Lahey Health at Home is seeking a Physical Therapist Case Manager for Beverly, MA. The 32-hour/day position covers Peabody and Lynnfield areas, with a day shift and full-time schedule.

The role involves coordinating care, initiating and adjusting the plan of care, and ensuring OASIS documentation is completed accurately and timely. A Massachusetts PT license and BLS are required.

Qualifications

• Degree from an accredited PT program; DPT preferred.
• Massachusetts Physical Therapist license is required.
• Active American Heart Association BLS certification.
• 2+ years of physical therapy experience, including home care/public health.
• OASIS experience is preferred.

Responsibilities

• Coordinate care for assigned patient caseload, delegating to/supervising PTA staff.
• Complete and document an initial patient/family assessment to determine home care needs.
• Develop patient-specific plan of care with goals and rehabilitation actions.
• Initiate and revise the plan of care as patient needs change.
• Communicate with physicians and obtain orders as required.
• Complete OASIS documentation in a timely manner.
