Job summary

Beth Israel Lahey Health is seeking a Physical Therapist to lead home care services, coordinating care, conducting initial assessments, and directing patient-specific plans of care within homes and communities in accordance with regulations and payer guidelines.

Responsibilities include delegating to PTA staff, updating care plans as patient needs change, and maintaining HIPAA compliance while ensuring timely OASIS documentation. A MA PT license and BLS certification are required.

Qualifications

• Licensed physical therapist in Massachusetts.
• Active BLS certification.
• OASIS experience is preferred.

Responsibilities

• Coordinate care for assigned patient caseload including delegating to and supervising PTA staff (if applicable), processing patient condition changes and completing OASIS time points.
• Complete and document an initial assessment of patient and family to determine home care needs and develop a patient-specific plan of care including goals.
• Initiate and revise the plan of care; administer therapies as prescribed and coordinate preventive/rehabilitative actions.
• Communicate with the physician regarding patient needs and changes in condition; obtain orders as required.
• Maintain confidentiality per HIPAA regulations and agency policies.
• Demonstrate proficiency in OASIS completion and timely documentation.
