Job summary

Beth Israel Lahey Health at Home in Burlington and Lexington Areas seeks a Physical Therapist to plan, organize, and direct home care services. You will compose a patient-specific plan, assess needs, and coordinate care with PTA staff as needed.

The role requires a PT degree, MA license, BLS certification, and at least 2 years in PT, with home care/public health preferred; OASIS experience is a plus. Competitive salary and benefits provided.

Qualifications

• Degree from an accredited program in Physical Therapy; DPT preferred.
• Current valid Massachusetts Physical Therapist license; Active American Health Association BLS certification; 2+ years of physical therapy experience, at least one in public health or home care; OASIS experience preferred.

Responsibilities

• Coordinate care for assigned patient caseload including delegating to and supervising PTA staff if applicable.
• Process patient condition change visits and complete OASIS time points.
• Complete and document initial assessment of patient and family to determine home care needs.
• Provide a complete physical assessment and develop a patient-specific plan of care with goals.
• Initiate and revise plan of care as patient status changes.
• Initiate preventive and rehabilitative therapy procedures.
• Administer medications and treatments as prescribed by physician.
• Communicate with physician about patient needs and changes; obtain orders.
