Job summary

AccentCare in California is seeking a dedicated Physical Therapist to deliver high-quality, in-home rehabilitation services. You will evaluate patient status, develop the plan of care and guide families in caring for therapeutic devices to maximize independence.

Responsibilities include direct therapy, timely clinical documentation, and adherence to our code of conduct within a supportive post-acute care team.

Qualifications

• Master's degree in physical therapy approved by CAPTE.
• Currently licensed to practice as a Physical Therapist in the state of operation.
• Home health/community experience preferred.
• CPR certification required; valid driver’s license and insured vehicle.

Responsibilities

• Assess and evaluate therapeutic/rehabilitative/functional status and participate in developing the total plan of care.
• Direct physical therapy treatment.
• Instruct patients, families and caregivers in the use and care of therapeutic appliances.
• Prepare and submit clinical notes in a timely manner.
• Adhere to the Company's code of conduct.
