Job summary

AccentCare, Inc. is seeking a Licensed Physical Therapist Assistant to provide patient care under the supervision of a Physical Therapist. You will manage a caseload, educate patients and families, and document treatments per policy.

The role covers Folsom, Placerville, Camino Hills, with a Monday–Friday 8am–5pm schedule and requires travel to multiple locations. You will work within an interdisciplinary team, ensure compliance with standards, and participate in agency in-services.

Qualifications

• Must hold an Associate degree in Physical Therapy from an accredited program.
• Must possess and maintain valid CPR certification.
• Ability to educate patients and families and document treatment accurately.
• Strong teamwork and communication with supervising therapist and interdisciplinary team.

Responsibilities

• Provide patient care and document treatment per physician orders and agency policy.
• Document interventions, approaches, and priorities for treatment.
• Educate and counsel patients and families in treatment plans and home safety.
• Report patient status to supervising therapist and team.
• Deliver skilled treatment within agency and payer guidelines.
• Collaborate with team to plan treatment and interventions.
• Complete daily notes and forms promptly to ensure uninterrupted care.
• Meet productivity standards and participate in meetings as assigned.
• Negotiate patient participation and adjust treatment plans as needed.
