Arlington Assured Home Health, a part of LHC Group within the Optum family, is hiring a Physical Therapist to assess patients in home health, develop treatment plans to improve mobility, and deliver interventions that enhance well-being in a home setting.

You will document progress, collaborate with care teams, and ensure plans comply with state guidelines. WA licensure and reliable transportation are required; CPR is preferred within 90 days of hire.

Competenze

• Current Physical Therapy licensure in the state of Washington.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Mansioni

• Evaluates the patient's functional status and therapy needs to inform plan of care.
• Develops a PT plan of care aligned with state laws and plan of care.
• Provides services within the scope of practice and coordinates with the health care team.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family about the plan of care and progress toward goals.
• Visits patients per the Plan of Treatment and submits progress notes.
• Completes all evaluations and reports outcomes per policy.
• Function effectively in diverse home settings and patient backgrounds.
